Target Group:
Adults aged 18+ experiencing social isolation, reduced independence, or disengagement from services, including older adults and vulnerable adults with low-level, non-regulated support needs.
Price: £3,750 (6-Week Intensive)
Capacity: 8–10 Residents Supported
Deployment: One Community Outreach Practitioner
Delivery Hours:
Up to 40 hours Weekly, flexibly split across residents.
Delivery Focus:
Flexible engagement spanning home-based support, community access, and accompanied appointments.
Functional Independence:
Targeted support in appointment attendance, routine management, public transport confidence, community participation, and sustained independent living.
